Below are guidelines for requesting a donation from MOSI:

• Submit your request via the online donation form below or US Mail at least (8) weeks prior to the event date
• Your organization must be a non-profit with 501 (c)(3) tax exempt status. A member of the non-profit organization should complete and submit the donation request form. The email on file must be from the non-profit organization.
• Your organization must serve the Greater Tampa Bay Area
• Your organization must share our commitment to education-focused missions or activities, promote successful outcomes for youth or families and help build strong communities
• A maximum of one donation may be requested per organization per year
• Due to the high volume of donation requests, we are unable to accept fax, phone, or email requests
